Alopecia, or hair loss, has a wide variety of causes. When treated medically or when it has been proven to be irreversible, it can be resolved surgically. Depending on the extent and characteristics of the condition, various techniques can be applied, such as scalp flaps or, more recently, expanders.
Autografts are procedures in which cylindrical grafts are used to restore affected areas. This technique includes micrografts and minigrafts. Minigrafts contain between three and four hairs per graft, while micrografts contain two hairs per graft. For placement, we use two main methods:
The FUMM or strip technique consists of extracting a small strip of scalp from the occipital region to obtain the necessary follicular units. On the other hand, the FUE (Follicular Unit Extraction) technique allows micrografts to be extracted directly from the occipital region. This procedure is performed under local anesthesia and on an outpatient basis, which facilitates the patient’s recovery.
Regarding flaps, there are different types depending on the area and the goal of the treatment. Inferior pedicle flaps are usually transverse, while pre- and post-auricular superior flaps have their pedicle located in the temporo-parietal region, allowing for effective and aesthetic coverage in selected areas.
Finally, expanders represent an innovative technique particularly indicated for cases of medial or partial alopecia due to burns. This method involves stretching hair-bearing areas using tissue expanders, facilitating the subsequent repositioning of the scalp and the gradual removal of hairless areas.
